Introduction

Appointments & Cancellations

  • Appointments must be scheduled in advance.
  • Clients are encouraged to arrive 10 minutes before their scheduled appointment.
  • Please come to your appointment with intake paperwork, either filled out on the website or printed and filled out.
  • If you are unable to make your appointment, please notify us 24 hours in advance. Late cancellations or no-shows will be subject to a fee.

Treatment Policies

  • Prior to any treatment, a comprehensive consultation will be provided to determine the best course of action for the client’s aesthetic goals.
  • Clients must complete and sign an informed consent form prior to receiving any treatment.
  • Trained and certified professionals will carry out all treatments.
  • We reserve the right to refuse treatment if it is deemed unsuitable for the client or if the client is uncooperative.

Pre-Treatment Guidelines

  • Avoid sun exposure or tanning beds for at least two weeks before treatment.
  • Before your treatment, please inform our staff about any medications, allergies, or health conditions.
  • For medical aesthetic patients, please come to the appointment with no makeup and clean skin. We will clean your skin again before any procedures. This step is necessary for JC and her staff to assess your skin.

Post-Treatment Care

  • Following any treatment, specific aftercare guidelines will be provided. Adherence to these guidelines is crucial for best results and to avoid complications.
  • Clients are urged to contact Hazelnut Aesthetics immediately for any signs of complications or unexpected reactions.

Payment & Refunds

  • Full payment is expected at the time of service.
  • We accept various payment methods, including credit/debit cards and cash.
  • Due to the nature of our treatments, we do not offer refunds. However, if you experience issues with the service provided, please discuss them with our team.

Client Confidentiality

  • We maintain the strictest confidentiality with all client records and treatment information. Personal and medical information will not be shared without written client consent.

Special Promotions

  • Hazelnut Aesthetics occasionally offers promotions or discounts. These cannot be combined with other offers or redeemed for cash.

Hormone Replacement Therapy (HRT)

  • Only qualified medical professionals will administer and oversee HRT.
  • A comprehensive consultation and medical evaluation are mandatory before starting HRT.
  • Regular follow-up appointments will be scheduled to monitor and adjust hormone levels as necessary.
  • Patients will be given a lab order form to complete for hormone replacement. Following the consultation appointment and before undergoing treatment, it is important for women to have their lab tests completed. Additionally, they must have a current mammogram report to be added to their chart records.

  • Client feedback is valuable and helps us continuously improve. We encourage clients to share their experiences, concerns, and suggestions.
